the Position Job Title: Neuro Oncology Care Manager Profession:
Registered Nurse Specialty: Oncology Duration: 13 weeks Shift: Day
Hours per Shift: 8 Experience: 2 years of Case Manager experience
and experience with Oncology License: RN Certifications: Required -
PALS or PEARS. Preferred - CPHON Must-Have: Experience with Neuro
Oncology patients Knowledge of Chemotherapy and its interactions
Ability to customize road maps per patient Experience in
coordinating follow-up appointments Experience in ordering and
following up on labs, appointments, scans, and referrals Experience
in authorization and DME ordering/coordination Competence in care
coordination and transition planning Strong needs assessment skills
Description: Responsible for developing customized road maps for
Neuro Oncology patients. Accountable for patient assessment,
implementation of physician orders, and treatment plans. Engaged in
monitoring and evaluating patient care processes and outcomes.
Coordinates activities with other services and disciplines as part
of a multidisciplinary team. The role requires a solid background
in Oncology care management. Experience in discharge planning and
concurrent review is essential. Knowledge of medical necessity and
evidence-based guidelines is necessary. Familiarity with patient
care standards and regulatory requirements is expected. Previous
experience in pediatric and adolescent care is preferred. Must
float to cover for colleagues as necessary. Participation in the
development and review of patient care plans is required.
