Job Description:
SUMMARY Demonstrating a heart of service, compassion, and
personal integrity consistent with the values of the Los Angeles
Mission, the Associate Case Manager / Outreach Worker plays a vital
role in guiding individuals from crisis toward stability and
transformation. This dual-role position is responsible for either
providing case management services or conducting community outreach
efforts. While some operational collaboration is expected, the
Associate Case Manager and Outreach Worker have distinct roles with
clearly defined responsibilities. I. ASSOCIATE CASE MANAGER The
Associate Case Manager is responsible for the coordination and
delivery of direct services to program participants experiencing
homelessness, addiction, trauma, or other barriers to stability.
The Associate Case Manager works collaboratively within a
multidisciplinary team to support individualized service planning,
resource connection, and long-term stabilization. Essential Duties:
Case Management & Support Planning Develop individualized SMART
goals and service plans tailored to each participant's needs.
Conduct bio-psychosocial assessments, treatment planning, progress
reviews, and discharge planning. Utilize evidence-based practices
such as Motivational Interviewing, Critical Time Intervention, and
trauma-informed care. Provide crisis intervention and conflict
resolution to stabilize participants in distress. Ensure compliance
with external mandates (e.g., probation, DMH, court orders) through
appropriate coordination. Service Navigation & Collaboration
Collaborate with internal SUD counselors, therapists, peer mentors,
and external providers. Facilitate linkages to housing, employment,
healthcare, substance use treatment, and education. Serve as the
participant's advocate with service providers, legal systems, and
benefit agencies. Lead or co-lead recovery-oriented life skills or
educational groups as assigned. Documentation & Compliance Maintain
accurate and timely documentation in Client Track and participant
charts. Conduct data entry, reporting, and recordkeeping in
compliance with funder and programmatic standards. Assist with
outcome tracking, goal achievement metrics, and resource
distribution. Spiritual Support (as appropriate) Offer prayer and
spiritual encouragement when welcomed by participants. Integrate
moral and ethical support rooted in grace, accountability, and
hope. Attend weekly chapel and staff prayer meetings to foster
alignment with Mission values. II. OUTREACH ASSOCIATE The Outreach
Associate serves as a frontline ambassador of hope and hospitality
to community guests accessing meals, showers, and emergency
services. This role aims to build trusting relationships, promote
available support services, and enhance the welcoming environment
at Los Angeles Mission. Essential Duties: Engagement & Advocacy
Actively engage individuals in the lobby and dining areas,
welcoming them with dignity and encouragement. Promote case
management services to community guests during mealtimes and shower
access periods. Build rapport with guests through consistent,
compassionate interaction and active listening. Foster a peaceful
and uplifting atmosphere in shared spaces, mitigating conflicts and
redirecting negative behavior when necessary. Community & Provider
Partnerships Develop relationships with external service providers
(e.g., medical clinics, MAT programs, legal aid, tax services,
voter registration, etc.). Coordinate on-site resource fairs and
schedule guest agencies to provide tabling and outreach. Maintain a
resource calendar and contact database to facilitate ongoing access
to community-based services. Lobby Oversight & Hospitality Serve as
a visible and supportive presence in the lobby to ensure safety,
structure, and order. Collaborate with security and other staff to
de-escalate disruptive behavior and guide guests toward respectful
engagement. Monitor supply distribution (e.g., hygiene kits,
resource flyers) and identify trends in guest needs. Spiritual
Climate Reflect the Mission's heart for restoration and compassion
through everyday interactions. Model patience, grace, and respect,
uplifting the dignity of every person served. Participate in
Wednesday chapel services with Mission participants and staff.
